Welcome from the School Nurses 

Dear Parents and Guardians,
Welcome to a new school year at Mount Pleasant Cottage School UFSD!
School Nurses -  Brianne Bohrman and Martin Dadzie are proud to serve as your child’s District School Nurse.  They look forward to partnering with you to ensure your child’s health, safety, and well-being throughout the school year.

Please take a moment to review these important health guidelines and reminders:

Required Health Forms & Medical Documentation

  • All annual physicals and up-to-date immunization records must be submitted to the Nurse’s Office in compliance with New York State law.
  • If your child has experienced any recent health updates, such as injuries, accidents, surgeries, or new diagnoses, please notify the health office promptly so we can support your child’s needs appropriately.

Medication Administration

  • If you wish to give the school nurse permission to administer scheduled or over-the-counter (OTC) medication to your child during school hours, you must submit a Medication Authorization Form. This form was included in your new student packet.
    • If you did not receive the form, a copy is available for download at the bottom of this web page.
  • It is your responsibility to supply all medications in their original prescription or OTC containers.
    • Medications will not be administered if the drug name, dosage, form, expiration date, or your child’s name is missing or illegible.

When to Keep Your Child Home

To help prevent the spread of illness, students should remain at home if they exhibit:

  • A fever of 100.4°F or higher
  • Vomiting or diarrhea
  • Symptoms of contagious illness (such as persistent cough, sore throat, or rash) Children must be symptom-free for at least 24 hours without the use of medication before returning to school.

Please ensure that your emergency contact details are up to date and accurate. This allows us to reach you immediately in the event of an emergency.
